How about the Health/Life? I know there's no Health/Life bar on screen since the game has no hud, but can't the Life code be detected and tampered? From what I heard this game has a PERMADEATH mechanic, so it's important to have an Infinite Health/God Mode option, so you won't have to start from scratch.

The game erase automaticly The save game! Can't be done! Need a Trainer for it.

This game definitely need a trainer for it. The disease will affect some of the area each time u failed the mission. Once the affected area reach the main character head then boom. All of your progress will be deleted.
